after you fill out the form online, please proceed to Payments section below.
OFFLINE
Please download and print one of the forms (see attached files section below) and fill it out.
Note that this form is also available at the rectory, at the OLM Church at the bulletin shelves, or at the Religious Ed office.
Once completed, you may send it back through one of the following methods.
- drop it at the parish office (rectory) mailbox
- email it as an attachment to [email protected]
- fax the completed form to 650- 755- 6704
- mail the form via post office to:
